Graft Material Or ADM To Support Breast Implant Revision Surgery

Today we’re going to talk a little bit about graft material. Maybe you’ve heard some things about the doctor’s going to do an internal lift or an internal bra. What does that mean? It means that the doctor’s going to be putting a piece of material in you, which is called ADM. ADM is acellular dermal matrix. What is that? That’s some form of graft material. It can be made of collagen, it can be made of all different types of products, Galatea, GalaFLEX we use. There’s other products called AlloDerm, which in the older times were used.

But in any case, sometimes you’ve had multiple revisions and there’s not enough capsule or tissue to tighten up a fold or to reposition the implant, or we need tissue coverage. We can use ADM graft or the Acellular Dermal Matrix graft. I like Galatea or GalaFLEX, which we can reshape it, cut it, and suture it right into the breast. It can be sutured into along the inframammary fold. For bottoming out it can be sutured along the inner breast if you have rippling or if you have symmastia.

But in any case, I like to start with the capsule or sling. It’s using the patient’s own capsule, but unfortunately, sometimes there’s not enough capsule or it’s not thick enough to hold up an implant. And then we have to use the graft material, suture it right into the periosteum of the rib so it secures it. And over time, for example, GalaFLEX can take up to a year to absorb, but once it does, it incorporates collagen and scar tissue and it helps to keep the fold up. So those are the main points with ADM graft.
